Founded in 1997 as Fox Cities Community Health Center, their teams provide free medical and dental care for residents of Outagamie, Winnebago and Calumet counties. In the mid-2000s, FCCHC was rebranded with the guidance and creative energy of Chief Idea Officer, Nate Wielgosh to Partnership Community Health Center. 

Creating a welcoming look, an inviting and friendly name increases the inclusivity of their patients and has grown into a multi-site full-service medical and dental clinic.
